Case Report: Undetectable Atherogenic Markers in a Lean Mass Hyper-responder).
This case report documents a unique clinical profile of a lean mass hyper-responder under intermittent fasting, characterized by the simultaneous undetectability of three atherogenic markers: large VLDL particles (<0.8 nmol/L), small LDL-P (<90 nmol/L), and an LP-IR score <25. Laboratory analysis was performed through an NMR LipoProfile by LabCorp (July 2025) and complemented with body composition data obtained by multifrequency bioimpedance (InBody 570). The patient, a 40-year-old male, exhibited a skeletal muscle–to–fat mass ratio of 18.4:1 and a total body water percentage of 73.0%, both unprecedented values in current scientific and athletic literature. These findings challenge the conventional interpretation of elevated LDL-C levels by highlighting the protective metabolic adaptations induced by intermittent fasting, including growth hormone–mediated muscle preservation, enhanced fatty acid mobilization, and ketone production. The report provides evidence that high LDL-C in this specific physiological context does not necessarily imply increased cardiovascular risk, but rather reflects a functional state of lipid metabolism. This case contributes novel data to the emerging field of personalized lipidology and may inform future research on metabolic health, nutrition, and risk stratification. In recognition of this exceptional physiological profile, the author has been officially awarded three world record titles by the World Records Union (WRU), underscoring the unprecedented nature of the findings. “Supporting files include original InBody 570 bioimpedance report and LabCorp NMR LipoProfile results for validation.”
