SCREENING OF KNOWLEDGE, AWARENESS, AND MEDICATION ADHERENCE OF ANTIDEPRESSANT AMONG PATIENTS IN REGION OF DEHRADUN

Background: Depression is a growing public health concern globally, including in India. Despite the availability of effective pharmacological treatments such as antidepressants, poor medication adherence and lack of awareness among patients remain significant barriers to successful management. This study was conducted to assess the levels of knowledge, awareness, and adherence to antidepressant medications—particularly Fluoxetine—among patients with depression in the Dehradun region of Uttarakhand. Objectives: To evaluate patient knowledge and awareness about antidepressants, measure medication adherence levels, and identify factors contributing to non-adherence in patients diagnosed with depression. Methods: A descriptive, cross-sectional, questionnaire-based survey was conducted on 120 patients aged 18–65 years who were diagnosed with Major Depressive Disorder and prescribed antidepressants. Data were collected from outpatient departments of government hospitals, private clinics, and pharmacies in Dehradun using a structured questionnaire, including the Morisky Medication Adherence Scale (MMAS-8). Statistical analysis was performed using SPSS version 25, with chi-square tests applied to assess associations between knowledge, awareness, and adherence. Results: The study found that while 80% of participants were aware that antidepressants treat depression, only 48% understood they are non-addictive, and 40% were aware that abrupt discontinuation may lead to relapse. Only 28.3% of patients demonstrated high adherence to prescribed medications, while 40% had medium and 31.7% low adherence. Significant factors associated with higher adherence included education level, regular follow-up, awareness of side effects, and prior discussion about treatment duration. Common reasons for non-adherence included forgetfulness (51.2%), improvement in symptoms (41.8%), and side effects (34.9%). Conclusion: This study highlights significant gaps in patient knowledge and adherence to antidepressant therapy in Dehradun. Targeted educational interventions, improved patient-provider communication, and active involvement of pharmacists are essential to enhance adherence and treatment outcomes. A region-specific and culturally sensitive approach is recommended to address stigma, improve mental health literacy, and strengthen the continuity of care.
