Prevalence and Associated Factors of Menopausal Insomnia and its Impact on Women's Quality of Life

This study examined the prevalence, predictors, and quality of life (QoL) impact of insomnia among 320 menopausal women (aged 45-60) using validated instruments (ISI, MRS, WHOQOL-BREF). Results revealed 58% prevalence of clinical insomnia (ISI≥15), with postmenopausal women showing significantly higher rates than perimenopausal women (62% vs 52%, χ²=4.87, p=0.027). Vasomotor symptoms were the strongest predictor (AOR=2.8, 95%CI:1.9-4.1, p<0.001), followed by anxiety (AOR=2.1, 95%CI:1.5-3.0, p=0.002) and sedentary lifestyle (AOR=1.7, 95%CI:1.2-2.5, p=0.008). Active women demonstrated 50% lower insomnia risk than sedentary counterparts (32% vs 64%, χ²=18.2, p<0.001). Insomnia severity correlated strongly with reduced Qol across physical (54.3±13.8 vs 72.5±11.2, p<0.001), psychological (48.9±12.1 vs 68.4±10.5, p<0.001), and social domains (60.1±11.4 vs 75.2±9.8, p<0.001). The findings highlight the multifaceted nature of menopausal insomnia, involving biological (hormonal changes, vasomotor symptoms), psychological (anxiety), and behavioral (physical inactivity) factors. These results support integrating sleep health into menopausal care through routine screening, targeted CBT-I interventions, and lifestyle modification programs. Policy recommendations include developing clinical guidelines for insomnia management during menopause, while research should explore culturally-adapted interventions for diverse populations. The study underscores the need for multidisciplinary approaches to address this prevalent yet under-treated condition that significantly impacts women's health and well-being during midlife.
