Surgical treatment of oroantral communication with PRF

Introduction: Oroantral communication (OAC) is a condition where there is an abnormal connection between the oral cavity and the maxillary sinus, which can lead to complications such as sinusitis or oroantral fistula. Objective: The aim of this study is to demonstrate the effect of better healing and closure of OACwith the application of platelet-rich fibrin (PRF). Material and Method: At the Clinic for Oral Surgery in Skopje, a 35-year-old patient, after clinical and radiographic examinations, was diagnosed with the presence of OAC, created after the extraction of the upper left first molar. Clinical examinations were performed using the Valsalva maneuver. The surgical treatment was performed using PRF.The surgical treatment included curettage of the alveolus and rinsing of the extraction wound with saline solution, applying two PRF plugs in the extraction wound and two PRF membranes over them, followed by suturing,. Results: Follow-up examinations were conducted on days 1, 7, and 14 after the surgical intervention. Clinically, no signs of inflammation of the soft tissues were observed, and there was no presence of an oroantral fistula. Radiographic examination was performed after 2 months, which showed the formation of a new bony floor in the maxillary sinus and the restoration of the Schneiderian membrane. Conclusion: The application of PRF in the surgical treatment of OAC represents a significant advancement in oral surgery. The growth factors and numerous cytokines present in PRF play a significant role in the process of angiogenesis, collagen formation, and cell migration, which are crucial in the wound healing process, stimulating tissue regeneration and reducing the risk of potential complications. Keywords: Oroantral communication, PRF,surgical treatment.
