Investigation and analysis of exercise behavior stage in non-dialysis patients with chronic kidney disease

ObjectiveTo investigate and analyze the exercise behavior stage in non-dialysis patients with chronic kidney disease (CKD), and to provide evidence for exercise rehabilitation exercise in CKD.MethodsA total of 581 non-dialysis CKD patients at stage 1-5 who visited the Renal Outpatient Department of Beijing Friendship Hospital, Capital Medical University from March 1, 2021 to February 28, 2022 were included by convenience sampling method. General demographic data of the patients were collected, and the exercise behavior was investigated by the Godin Leisure-Time Exercise Questionnaire (GLTEQ). The differences in exercise behavior stage and demographic characteristics in CKD patients with various stages were compared.ResultsThere were 117 (20.14%) cases in the active stage, 163 (28.06%) cases in the moderately active stage and 301 (51.81%) cases in the insufficient stage. The GLTEQ scores in the CKD stage 1-2, stage 3, stage 4 and stage 5 were 20.46 ± 4.25 points, 17.29 ± 3.17 points, 13.83 ± 3.69 points and 12.54 ± 2.95 points, respectively. The proportion of active exercise was 56.64%, 18.32%, 6.92% and 4.67%, respectively; and the proportion of insufficient exercise was 19.47%, 40.10%, 70.44% and 80.37%, respectively. There were significant differences in the GLTEQ scores and the stage distribution of exercise and exercise behavior in CKD patients at various stages (P<0.05). There were significant differences in the GLTEQ scores among different ages [18-35 years old (21.60 ± 3.35) points; >35-50 years old (17.24 ± 3.14) points; >50-65 years old (12.38 ± 2.79) points], genders [male (18.53 ± 3.85) points; female (12.84 ± 3.00) points], CKD course [<3 years (21.37 ± 3.48) points; 3-5 years (17.28 ± 3.32) points; >5 years (12.86 ± 3.01) points], primary incidence of CKD[glomerulonephritis (12.18 ± 2.73) points; hypertensive renal damage (15.80 ± 2.50) points; diabetic renal disease (20.91 ± 4.08) points; chronic pyelonephritis (17.93 ± 0.68) points; others (20.05 ± 0.23) points], education level[junior high school or below (20.85 ± 3.91) points; High school or technical secondary school (16.69 ± 2.40) points; junior college or above (12.37 ± 2.78) points] and residence status [urban residents (13.79 ± 2.92) points; suburban or rural residents (21.31 ± 2.76) points] (P<0.05).ConclusionsThe overall exercise activity of non-dialysis CKD patients is insufficient, and the worse the renal function indicates a more obvious exercise deficiency. It is necessary to provide targeted exercise guidance according to the differences in CKD stage, age, gender, course of disease, primary disease, education level and urban and rural living conditions.
