Introduction: Fat embolism is a complication of polytraumatized patients that is rarely suspected. It is usual for medical examiners as well as clinicians to consider fat embolism as a final cause of death when there is no other injury regardless of its entity, but not in patients with a minimal injury. Case report: A 22-year-old man with no relevant pathological history was admitted after a traffic accident as a motorcycle driver. He presented with left fist trauma with radius and ulnar styloid closed fracture. Initially, there were no elements that would lead to suspect fat embolism syndrome. The autopsy showed edema and fat embolism in lungs, in the absence of another cause of death. Conclusion: Medical examiners and traumatologists should suspect fat embolism as a cause of death even in the presence of minimal trauma fractures and in patients with no symptoms of fat embolism syndrome.
