A triple A syndrome with neurological findings; c464G>A mutation in the AAAS gene = Tripla A szindróma neurológiai leletekkel;c464G>A mutáció az AAAS génben

Introduction – Allgrove syndrome ortriple A (3A) syndrome is a multisystemdisorder classically defined as the triad ofesophageal achalasia, alacrimia and adrenalinsufficiency due to adrenocorticotropichormone insensitivity. Approximately onethird of patients experience neurologicaldysfunction, including peripheral andautonomic nervous system dysfunction, leading some authors to use the term 4Asyndrome (achalasia, alacrimia, adrenalinsufficiency and autonomic abnormalities).Since its first description in 1978, knowledgeof its clinical and genetic features hasincreased; however, the current literature islimited to case reports and case reviews. Methods – A 20-year-old male patient wasadmitted to the clinic with the followingcomplaints: difficulty in walking, tinglingsensation in the feet and weakness of1.5 years’ duration. He had undergoneendoscopy and balloon dilatation surgery 2years previously. Results – We performed whole exome analysis on the patient and detected the c464G>Ap.(Arg155His) variant in the AAAS gene inhomozygous form. It was interpreted as ‘pathogenic’ according to the ACMG 2015 criteria:homozygous pathogenic variants in this genecorrespond to the phenotype ‘AchalasiaAddisonism-Alacrimia’ (OMIM:231550).Conclusion – We present this case todraw attention to the fact that patients maypresent with late-onset neurological findingswithout the classic Allgrove syndrome triad. Módszerek – Egy 20 éves férfi beteg került a klinikára a következő panaszokkal: járási nehézség, bizsergő érzés a lábakban és 1,5 éve tartó gyengeség.
