Correlation between the urinary albumin creatinine ratio and periodontal clinical indicators in type 2 diabetes mellitus patients

ObjectiveTo explore the correlation between urine albumin creatine ratio (UACR) and periodontal clinical indicators in patients with type 2 diabetes mellitus (T2DM), thus providing more clinical evidence for early diagnosis of diabetic kidney disease (DKD).MethodsA total of 150 T2DM patients hospitalized in the First Central Hospital of Baoding City from November 2023 to February 2024 were retrospectively included in the present study. Based on the UACR levels, patients were divided into three groups: normal albuminuria group (UACR<30 mg/g), micro albuminuria group (UACR 30-300 mg/g), and macro albuminuria group (UACR>300 mg/g), with 50 patients in each group. General clinical data, biochemical indexes, and periodontal clinical indicators like the bleeding index (BI), periodontalpocket depth (PD), clinical attachment loss (CAL), and number of missing teeth were compared among the three groups. The estimated glomerular filtration rate (eGFR) was calculated.ResultsThere were significant differences in the levels of interleukin 6 (IL-6), serum creatinine (Scr), eGFR, and periodontal clinical indicators among the three groups (P<0.05). Patients in the microalbuminuria and macroalbuminuria groups exhibited significantly higher levels of IL-6, Scr, BI, PD, CAL, and number of missing teeth compared to the normoal buminuria group, while their eGFR levels were significantly lower (P<0.05). Furthermore, patients in the macroalbuminuria group demonstrated significantly elevated IL-6, Scr, PD, CAL, and number of missing teeth, along with lower eGFR levels than the microalbuminuria group (P<0.05). Spearman correlation regression analysis revealed positive correlations between UACR and periodontal clinical indicators (P<0.05). The binary logistic regression model indicated that periodontal clinical indicators were independent risk factors for proteinuria in T2DM patients (DKD) (P<0.05). Receiver operating characteristic (ROC) curve showed that the area under the curve (AUC) of BI, PD, CAL, and number of missing teeth in predicting DKD was 0.722 (P<0.001), 0.911 (P<0.05), 0.924 (P<0.05), and 0.862 (P<0.05), respectively.ConclusionPeriodontal clinical indicators are positively correlated with UACR, which is an independent risk factor for the occurrence of urinary albumin, and it can be used as an important clue for the early diagnosis of DKD.
