Importancia de la anatomía radiográfica tridimensional en cirugía apical.: Enseñanzas de un caso

Background: Cone Beam Computed Tomography (CBCT) has revolutionized endodontic surgery by providing detailed three-dimensional images that enable better diagnosis, planning, and postoperative evaluation. Objective: To evaluate the effectiveness of CBCT in endodontic surgery, highlighting its benefits in visualizing complex pathologies and treatment planning. Case Description: A 35-year-old female patient presented with an active fistula in the vestibular area of the first dental quadrant, with a history of failed endodontic treatments. CBCT revealed the presence of an included root remnant from dental organ 14, not visible in conventional radiographs. Apical surgery was performed with foreign body removal and tetracycline detoxification, achieving complete fistula resolution. Conclusions: CBCT proved to be a fundamental diagnostic tool, enabling the identification of a recurrent fistula's cause not visible in conventional radiographs. Three-dimensional visualization facilitated precise surgical planning, confirming its importance in complex endodontic cases.
