This study analyzes early general clinical and laboratory predictors of mortality in patients with purulent-septic soft tissue complications of the lower extremities against the background of type 2 diabetes mellitus. The work examines 64 patients, stratified by type of central hemodynamic disturbance: hypovolemic, cardiac, distributive, and microcirculatory types. Key biomarkers such as procalcitonin (PCT), lactate, NT-proBNP, and glucose levels were assessed in correlation with SOFA score progression and mortality outcomes. The total lethality rate was 18.75%, with higher rates observed in patients with distributive and microcirculatory failure. Elevated levels of lactate (>3.7 mmol/L), NT-proBNP (>5000 pg/mL), and procalcitonin (>8 ng/mL), combined with leukocytosis (>16×10⁹/L) and hyperglycemia (>10.8 mmol/L), were significant predictors of poor outcomes. The study highlights the prognostic importance of systemic inflammatory response and early hemodynamic destabilization in septic diabetic patients.
