Eficacia de los exosomas sintéticos aplicados con dermapen en el tratamiento de liquen escleroso vulvar: reporte de un estudio clínico e histopatológico

Vulvar Lichen Sclerosus (VLS) is a chronic, progressive inflammatory dermatopathy that affects women at various stages of life. Clinical presentations vary widely, ranging from hypo- or hyperpigmented lesions that begin on the vulva and may extend to the labia minora and the perianal region. The treatment of VLS poses a therapeutic challenge, with synthetic exosomes emerging as a promising therapeutic option. In this study, the efficacy of synthetic exosomes delivered via dermapen in promoting tissue regeneration in the vulvar region affected by VLS was evaluated, considering the clinical and histopathological features of a 34-year-old female patient, who presented with vulvar pruritus; gynecological examination revealed hardened, rough vulvar skin with scaling and depigmentation of the labia majora. A clinical diagnosis of lichen sclerosus was made, pending anatomopathological confirmation. The patient underwent monotherapy with synthetic exosomes (V-TECH SYSTEM) delivered via dermapen, treatment sessions were administered every ten days for a total of five sessions. Subsequent evaluation demonstrated symptomatic improvement, including reduced pruritus and hyperpigmentation, with overall favorable clinical changes in the vulvar region. It was concluded that exosomes were found to be effective in promoting the repair of inadequate or damaged reproductive tissues, alleviating perianal and inguinal pruritus, and reducing acanthosis, hyperkeratosis and inflammation in the labia majora. However, the results are not conclusive and the therapeutic challenge remains to conduct further studies to substantiate the use of exosomes as an alternative treatment for VLS.
