The Role of Tumor Treating Fields (TTFields) in the Management of High-Grade Gliomas
High-grade gliomas, such as glioblastoma, are aggressive brain tumors with limited treatment options and poor patient outcomes. Tumor Treating Fields (TTFields) have emerged as a novel therapeutic approach that uses alternating electric fields to disrupt cell division. This review synthesizes current evidence on TTFields, highlighting key findings from clinical studies. The EF-14 trial demonstrated that adding TTFields to maintenance temozolomide significantly improved progression-free survival (6.7 months vs. 4.0 months; HR 0.63, 95% CI 0.52-0.76) and overall survival (20.9 months vs. 16.0 months; HR 0.63, 95% CI 0.53-0.76) compared to temozolomide alone. Other studies suggest that TTFields can enhance the effectiveness of radiation therapy by increasing DNA damage in tumor cells. However, challenges such as patient compliance, cost, and variable tumor response remain. Future research directions include combining TTFields with noveltherapies, improving device technology, and expanding its use to other cancer types.
