SGNDV-001: disitamab vedotin with pembrolizumab in HER2-expressing locally advanced or metastatic urothelial carcinoma
Platinum-based chemotherapy for the treatment of locally advanced or metastatic urothelial carcinoma (la/mUC), has been the first-line standard of care for many decades. Enfortumab vedotin, an antibody-drug conjugate, combined with pembrolizumab, a programmed death 1 (PD-1) inhibitor, recently demonstrated improved efficacy versus chemotherapy in la/mUC. Since 60%-80% of patients with UC have tumors expressing human epidermal growth factor receptor 2 (HER2), HER2-directed vedotin-based antibody-drug conjugates may also be beneficial in la/mUC. The phase 3 trial SGNDV-001 (C5731001; NCT05911295) is evaluating disitamab vedotin (HER2-directed antibody-drug conjugate) with pembrolizumab compared with chemotherapy in treatment-naive patients with HER2-expressing la/mUC. Dual primary endpoints are progression-free survival (per blinded independent central review) and overall survival. Potential synergistic effects of disitamab vedotin and pembrolizumab could establish this combination as a novel therapeutic option for HER2-expressing la/mUC. Platinum-based chemotherapy is used for first-line treatment of locally advanced or metastatic urothelial cancer (la/mUC), a cancer that starts in the lining of the urinary tract and spreads to nearby tissue, lymph nodes, or other parts of the body. Recently, antibody-drug conjugates (ADCs), which are made up of an antibody attached via a linker to an agent that can kill cells, have become treatment options in la/mUC. About 60% to 80% of people with la/mUC produce a type of protein known as HER2 on their tumor. Treatments that target HER2 May therefore be beneficial. Disitamab vedotin, a type of ADC, targets HER2 on the surface of cells. Disitamab vedotin can then enter the cells and deliver an anticancer agent called MMAE directly to them. Combining disitamab vedotin with immunotherapy (treatment that enables a person’s own immune system to find and kill cancer cells) may work better than either drug by itself. The SGNDV-001 trial is evaluating disitamab vedotin combined with a type of immunotherapy (called pembrolizumab) in patients with tumors that express HER2 and who have not been treated for la/mUC before. The goal of this trial is to see if this combination can help delay the progression of the cancer and allow patients to live longer compared with patients receiving chemotherapy. Researchers are also looking at how safe this combination is. If the trial is successful, disitamab vedotin plus pembrolizumab could become a treatment option for this patient population.
