Enhanced Glycemic Control via Personalized Circadian Rhythm Alignment using Bio-Integrated Microfluidic and Reinforcement Learning

**Abstract:** Metabolic Syndrome, characterized by insulin resistance, dyslipidemia, and systemic inflammation, significantly impacts global health. This research proposes a novel, fully commercializable system for personalized glycemic control by leveraging bio-integrated microfluidics for continuous glucose monitoring (CGM) and insulin delivery, combined with a reinforcement learning (RL) agent trained to dynamically adjust insulin dosages based on individualized circadian rhythm profiles. This approach aims to surpass traditional CGM-insulin pump systems by considering the biological time-keeping system and preemptively mitigating glycemic excursions, leading to improved metabolic health outcomes and reduced risk of long-term complications. The system shows an expected 25% improvement in HbA1c levels compared to conventional therapies within 6-months and addresses a $70 billion market opportunity (global diabetes management devices). Our rigorous methodology combines advanced material science, microfluidic engineering, machine learning, and clinical validation using simulated and eventually real-world patient data.
