Contemporary Advances in Heart Failure Treatment: From Medications to Regenerative Therapy
Background: Heart failure (HF) remains a leading cause of morbidity, mortality, and healthcare utilization worldwide. Despite advances in medical therapy, the complex pathophysiology of HF demands continual innovation in treatment strategies. Objective: This review aims to summarize contemporary advances in the management of heart failure, focusing on recent developments in pharmacologic therapies, device-based interventions, and regenerative approaches. Methods: A comprehensive review of recent clinical trials, guideline updates, and emerging technologies was conducted, emphasizing treatments that have demonstrated meaningful impacts on patient outcomes. Results: Novel pharmacologic agents, such as sodium-glucose co-transporter 2 (SGLT2) inhibitors and angiotensin receptor-neprilysin inhibitors (ARNIs), have significantly improved survival and reduced hospitalizations in patients with heart failure with reduced ejection fraction. Advances in device-based therapies, including cardiac resynchronization therapy, implantable cardioverter-defibrillators, and left ventricular assist devices, have further expanded options for advanced heart failure management. Regenerative therapies, including stem cell-based interventions and gene therapy, offer promising avenues for myocardial repair, although clinical translation remains in early stages. Challenges persist, including underutilization of therapies, heterogeneity of patient phenotypes, and barriers to access, highlighting the need for personalized and multidisciplinary approaches. Conclusion: Contemporary treatment strategies for heart failure reflect a rapidly evolving and multidisciplinary field. Ongoing research into precision medicine, regenerative therapies, and digital health integration holds promise for further improving outcomes and altering the natural course of this complex disease.
