Cardiomyopathy: peculiarities of outpatient management of patients
Cardiomyopathies (CMP) are a heterogeneous group of myocardial diseases characterized by structural and functional changes in the heart muscle that are not attributable to coronary, hypertensive or valve pathology. The disease includes several subtypes: hypertrophic, dilative, restrictive, arrhythmogenic right ventricular cardiomyopathy and unclassified forms. Effective outpatient management of such patients requires a multidisciplinary approach, early detection of symptoms, continuous monitoring and individualization of treatment. The authors review a number of aspects of outpatient management of patients with CMP. For example, modern diagnostic methods are covered, including the use of echocardiography, cardiac MRI, genetic testing and biomarkers. Particular attention is paid to assessing the risk of disease progression and sudden cardiac death. Criteria for assessing the severity of the condition, predicting complications, and selecting treatment tactics are presented. Algorithms for monitoring patients in outpatient follow-up using remote technologies are discussed. The main groups of drugs used to treat CMP are reviewed, including beta-blockers, ACE/ARA inhibitors, mineralocorticoid receptor antagonists, antiarrhythmic agents, and drugs that affect myocardial remodeling. Modern therapies such as implantation of cardioverter-defibrillators, resynchronization therapy devices, and catheter ablation are described. Indications for heart transplantation and other surgical interventions are discussed. The paper emphasizes that outpatient management of patients with CMP should be aimed at improving the quality of life, preventing complications and reducing mortality. Practical recommendations for general practitioners, cardiologists and other specialists involved in the management of this group of patients are offered.
