Stress regulation in patients with diabetes mellitus with various dynamic types of disease course

The paper deals with psychological peculiarities of stress regulation in patients with diabetes mellitus. The leading role of self-regulation in the formation of systemic stress-induced hormonal failure as a common psychophysiological cause of diabetes mellitus is shown. On the basis of the description of the psychosomatic peculiarities in the course of diabetes mellitus, groups of patients with different dynamic types of this disease course have been identified. Depending on the group type, typological features of stress regulation characteristic of patients have been described. The motivation for building up a stress-limiting behaviour model as a set of coping strategies and psychological defenses used by them has been analyzed. Non-productive (pathogenic) models of under stress behaviour regulation, which can result in aggravating the course of the disease, have also been analyzed. The phenomenon of "reversible stress" has been described as a model of pathological stress regulation typical of patients with malignant rapid type of diabetes mellitus. Based on the pathogenic personality complex of characteristics identified in patients, which can be considered as psychological grounds for exacerbating pathology and the transition of the dynamics of diabetes mellitus into a malignant rapid type of course, comprehensive corrective recommendations for clinical psychologists of endocrinological departments have been formed.
