ROLE OF SUPRACRESTAL TISSUE ATTACHMENT (STA) IN IMPLANT SUCCESS—A REVIEW.
ABSTRACT The long-term success of dental implants depends on osseointegration as well as the stability and integrity of the peri-implant soft and hard tissues, which are all part of the Supracrestal Complex Attachment (STC/STA). The peri-implant mucosa, marginal bone, implant-abutment interface, and prosthetic emergence profile are all parts of this functional unit. Recent clinical paradigms underscore the Umbrella Concept—a holistic methodology that emphasizes the directed modification of the SC during surgical and prosthetic phases to improve tissue preservation and aesthetics. Evidence indicates that a thick STH (≥3 mm), subcrestal placement with platform-switching, and a concave prosthetic emergence profile significantly enhance marginal bone stability and soft tissue health. The Umbrella Concept combines these parts by shaping the alveolar crest during surgery and using pre-contoured provisional restorations. This leads to minimal bone remodeling (≤0.2 mm over 2 years), less inflammation around the implant, and better aesthetic results.
