Abstract The aim of this study is to analyze the impact of self-management on the health status of patients with Type 2 Diabetes Mellitus (T2DM) within the context of a Disease Management Program (DMP). Self-management includes independent blood glucose control, adherence to diet, physical activity, medication adherence, and psychological resilience. The paper presents data from contemporary research and DMP implementation outcomes, demonstrating that the development of self-management skills significantly improves clinical outcomes, reduces the risk of complications, and enhances patients’ quality of life.
