The Potomac River Air Tragedy: Inside the Special Flight Rules Area (SFRA) Failure

The rapid growth of mixed-use airspace operations near Washington, D.C., has exposed persistent vulnerabilities in civil-military coordination, surveillance coverage, and communication protocols. This research investigates the fatal mid-air collision involving Envoy Airlines Flight 5342 and a U.S. Army UH-60M helicopter on January 29, 2025, as a case study illustrating systemic gaps in the National Capital Region’s air traffic governance. Through analysis of FAA and DoD coordination failures, SFRA compliance inconsistencies, ATC audio, and existing radar surveillance limitations - including those posed by stealth-capable aircraft - this study highlights the disconnect between regulatory policy and operational execution. Additionally, the paper explores human factors, institutional responsibility, and training disparities that contributed to the breakdown. Findings underscore the urgent need for joint procedural reforms, data-sharing advancements, and simulation-based cross-agency training to prevent future incidents in high-density, high-security airspace.
