Why You'll Never Know What's Covered_ The Symbolic Collapse of U.S. Health Insurance
Main article Abstract This essay analyzes why patients, providers, and even insurers cannot determine what is “covered” within U.S. health insurance systems. Using the CODES framework (Chirality of Dynamic Emergent Systems), the piece explains how denial logic, symbolic drift, and probabilistic coding structures create institutional incoherence. The essay introduces Phase Alignment Score (PAS) as a coherence-based replacement metric and offers a practical schema for building resonance-aligned healthcare systems that restore transparency, agency, and trust.
