A Case Report: Kaposi Sarcoma of the Ankle Complicated By Emphysematous Osteomyelitis
Abstract Kaposi sarcoma (KS) is a malignant vascular condition that frequently occurs in patients with weakened immune systems, such as those with HIV/AIDS or those receiving immunosuppressive treatment. While KS usually appears as skin lesions, it can also occur in many parts of the body, including unusual areas like the ankle. The complications of KS are varied and can encompass severe infections, particularly in patients with impaired immune systems. This case report examines an uncommon and intricate manifestation of KS: a confined ankle lesion that advanced to emphysematous osteomyelitis, a severe bone infection marked by the presence of gas-producing microorganisms. In this report, we present a case of Kaposi's sarcoma (KS) that was found specifically in the ankle of a 70 -year-old patient with a weakened immune system. The condition was further aggravated by the presence of emphysematous osteomyelitis. Originally misdiagnosed as a persistent wound, the lesion was subsequently recognized as Kaposi's sarcoma (KS). The patient experienced emphysematous osteomyelitis as a result of infection caused by bacteria that produce gas. Despite undergoing extensive medical interventions such as debridement and administration of antibiotics, the patient did not achieve favourable results. This instance emphasizes the importance of being watchful in handling complicated cases that involve KS and severe infections. The diagnostic imaging, including magnetic resonance imaging (MRI), detected a mass on the ankle that is in line with Kaposi sarcoma. The diagnosis of KS was confirmed through histopathological investigation of a biopsy sample, which revealed the presence of abnormal spindle-shaped cells and increased blood vessel growth. Following a few weeks after the first diagnosis and therapy, the patient exhibited intense pain, edema, and indications of infection throughout the body. Imaging scans detected indications of bone infection, and subsequent diagnostic investigation indicated emphysematous osteomyelitis. The cultures obtained from the infected bone exhibited the growth of Clostridium species and other anaerobic bacteria, thereby establishing the existence of pathogens capable of producing gas. The patient's condition was exacerbated by widespread inflammation and indications of sepsis. This case emphasizes various crucial factors in the treatment of Kaposi sarcoma accompanied by serious infection consequences. Kaposi's sarcoma (KS), although mostly affecting the skin, can lead to severe consequences both in specific areas of the body and throughout the entire system, especially in individuals with weakened immune systems. The occurrence of emphysematous osteomyelitis in this situation is an uncommon and serious complication that greatly complicates the treatment and results.
