TyAr®-Prosthetic Bar Guide (All-in-Guide): Digital protocol for guided surgery with integrated prosthetic bar
This document presents the original development of the TyAr®-Prosthetic Bar Guide (All-in-Guide), an integrated digital protocol for the fabrication of surgical guides incorporating, in a single device, the provisional prosthetic bar derived directly from the design of a monolithic prosthesis. The CAD-CAM workflow includes: Initial prosthetic design in DentalCAD with FP1/FP2 provisional restoration. Guided surgical planning in Exoplan using straight MUA abutments and adapted channels. Conversion of the monolithic prosthesis into a passive bar with controlled tolerance (+100 µm). Boolean subtraction to integrate the bar housing into the surgical guide. Fabrication by 3D printing or milling of guide, bar, and provisional restoration. Clinical protocol: simultaneous insertion of guide and bar, with implants carrying self-locking abutments, leaving the bar fixed in the planned position for immediate seating of the FP1 restoration with passive fit. Innovation & advantages: Eliminates intermediate steps and reduces cumulative errors. Integrates prosthetic support and surgical guidance in a single device. Improves predictability and shortens surgical time. Suitable for immediate loading protocols such as All-on-4. This document constitutes the first public disclosure of the TyAr®-Prosthetic Bar Guide protocol, serving as a priority reference for any future citation or recognition of authorship.
