Missing links: Understanding blindness to safety risks

This document is the result of a joint effort by members of the ESReDA project group on Risk, Knowledge and Management. They are experts from different European countries who work in different roles related to safety management, from firms in different industry sectors, consultancies, safety authorities, and research and development organizations. The document aims to provide useful information, from both theoretical and practical viewpoints, about the intersection between risk management, knowledge management and decision-making concerning safety issues, based on current practices and the state of scientificknowledge. The members of the project group on Risk, Knowledge and Management and their organizations at the time of their contribution to the group’s work are listed below:• John Kingston (NRI Foundation, The Netherlands)• Myrto Konstantinidou (NCSR Demokritos, Greece)• Peter Kouwenhoven (expert consultant Novartis, The Netherlands)• Eric Marsden (FonCSI, France)• Sever Paul (AGIFER, Romania)• Dan Serbanescu (Nuclearelectrica SA, Romania)• Zdenko Šimić (EC JRC, The Netherlands)• John Stoop (Kindunos, The Netherlands)• Tuuli Tulonen (Tukes, Finland)• Frank Verschueren (Ministry of Labour, Belgium – retired)• Ana Lisa Vetere Arellano (EC JRC, Italy)• Maria Luisa Villani (ENEA, Italy) This project group work was coordinated successively by Zdenko Šimić, Tuuli Tulonen, Eric Marsden and Myrto Konstantinidou.
