Integrasi Analisis Bow-Tie dan Dynamic Risk Assessment Untuk Manajemen Risiko Keselamatan Kerja: Studi Kasus Pada PT. Sumber Graha Sejahtera
ABSTRACT The wood processing industry has a high level of occupational safety risk and requires a management system that goes beyond static risk analysis. This research aims to develop an integrated risk management framework that combines Bow-Tie Analysis with Dynamic Risk Assessment (DRA) to create a proactive monitoring system. A case study was conducted at PT. Sumber Graha Sejahtera, a wood processing company in Indonesia. The initial risk analysis identified 68 risk variables, of which 28 were categorized as "extreme" and became the main focus. Bow-Tie analysis was used to map risk scenarios, identify threats, consequences, and barriers for these risks. The results of this analysis then became the basis for designing a DRA model by translating each qualitative barrier into measurable Key Performance Indicators (KPIs), complete with a three-color threshold monitoring system (Green, Yellow, Red). The research results show that the majority of incidents were caused by failures in preventive barriers, especially in human factors (non-compliance with SOPs) and technical factors (lack of machine protection). The DRA framework designed provides a proactive monitoring mechanism to assess the effectiveness of barriers in real-time, enabling management to take corrective action before incidents occur. This integrated approach has proven to change the safety management paradigm from reactive to proactive, offering a practical model for high-risk industries. Keywords: Bow-Tie Analysis, Dynamic Risk Assessment (DRA), Occupational Safety, Risk Management, Wood Processing Industry, Key Performance Indicators (KPIs).
