Evaluation of placenta accreta spectrum disorders Using Prenatal ultrasound staging system

Background: Placenta accreta spectrum (PAS) disorders represent a diverse array of conditions characterized by anomalous trophoblastic tissue adhesion or invasion into the myometrium. Recent advancements in prenatal imaging modalities and a refined understanding of the natural progression of these conditions have contributed to a notable rise in the prenatal detection rate of PAS.Objectives: To establish an ultrasound-based staging system for PAS disorders and assess its correlation with surgical outcomes and the clinical staging system for PAS disorders as proposed by FIGO.Methods: This investigation took place within the Department of Obstetrics and Gynecology at Al-Azhar University Hospitals following the approval of the Research and Ethics Committee. The study was carried out from March 2021 to March 2023, during which a total of twenty (n=20) pregnant individuals at 32–34 weeks of gestation and presenting with placenta previa. Results: Investigating (PAS) disorders, this study evaluated the association between PAS disorder stages and surgical outcomes. Findings revealed operative and postoperative data, including mean blood loss, units of blood components transfused, operation time, and hospital stay. Notably, correlations were identified between the Placenta Accreta Imaging System (PAIS) score and parity, blood transfusion, and FIGO Grade 2 and 3.Conclusions: The study concluded that the proposed ultrasound and clinical scoring system holds significant predictive value for Morbidly Adherent Placenta (MAP).
