Evaluation of Urinary Activin A in different stages of AKI

Background: Acute kidney injury (AKI) is a critical and potentially fatal condition with complex pathophysiological features.Aim: To assess the role of urinary Activin A in different stages of acute kidney injury.Patients and methods: This case series research was performed on ninety cases with acute kidney injury who were distributed according to the stage of AKI into three groups, stage one acute kidney injury (19 cases), stage two acute kidney injury(23 cases) and stage III (48 cases).Patients were selected from patients admitted in the ICU, Al-Hussein hospital in Cairo between April 2023 and October 2023.Results: The Human Activin A level was statistically significantly greater in stage three acute kidney injury compared with stage one and stage two acute kidney injury. Moreover, the Human Activin A level was statistically significantly greater in stage 2 AKI as compared to stage 1. In all stages of AKI, there was a greatly statistically significant positive association between Human Activin A and serum creatinine. The best cutoff point of human activin A to identify cases with stage 3 AKI from stage 2 AKI was>156.47 with 91.7% sensitivity and 87% specificity. The area under the curve was 0.945 with a high statistically significant value (p-value lower than 0.001).Conclusion: The research found a greatly significant positive association between urinary Human Activin A and serum creatinine, suggesting it can be used as a marker for diagnosing and monitoring the severity of AKI.
