Pediatric Venovenous Extracorporeal Membrane Oxygenation Flow-related Problems and Management: A Single-center Study
Background: Venovenous extracorporeal membrane oxygenation (VV ECMO) is a critical intervention for pediatric patients with refractory respiratory failure. However, flow-related complications pose significant risks. This study examines the incidence, management, and outcomes of flow-related events in pediatric VV ECMO at a tertiary care center. Methods: A retrospective cohort analysis was conducted on 11 pediatric patients (aged 1 month-18 years) supported with VV ECMO between 2018 and 2023.Data included demographics, extracorporeal membrane oxygenation (ECMO) parameters, flow-related events (e.g., thrombosis, hemolysis, cannula malposition), interventions, and clinical outcomes. Results: Flow-related events occurred in 45% of patients, with thrombosis (40%), low flow in 40%, and pump failure in 20%.Interventions included circuit component replacements (20%), anticoagulation adjustments (60%), and cannula repositioning (40%).Conclusion: Flow-related events during pediatric VV ECMO are common and associated with increased mortality.Proactive monitoring and timely interventions may improve outcomes.
