Breaking the Pattern: Unmasking a Rare Form of Cushing Disease
A bstract Cushing’s disease is a rare cause of Cushing syndrome resulting from an ACTH-secreting pituitary adenoma. It may present with overlapping clinical features and conflicting diagnostic tests. It is most often characterized by partial suppression of cortisol with high-dose dexamethasone. However, in rare cases, cortisol levels remain non-suppressible even with supraphysiological doses. We present a case of pituitary-dependent hypercortisolism with absent suppression on high-dose dexamethasone, highlighting the diagnostic challenges and implications for management.
