A Comparative Study Of Pericapsular Nerve Group (Peng) Block Versus Quadratus Lumborum (Qlb) Block For Postoperative Analgesia In Total Hip Arthroplasty
Background: Total Hip Arthroplasty (THA) is associated with significant postoperative pain, which necessitates effective multimodal analgesia to facilitate early mobilization and reduce opioid consumption. This study aimed to compare the analgesic efficacy and functional outcomes of the Pericapsular Nerve Group (PENG) block versus the Quadratus Lumborum (QLB) block as part of a multimodal analgesic regimen in patients undergoing THA under spinal anesthesia. Methods: A prospective, randomized, controlled study was conducted on 40 patients scheduled for THA. Patients were randomly allocated to two equal groups (n=20): Group PENG received an ultrasound-guided PENG block, and Group QLB received an ultrasound-guided anterior QLB, both utilizing 20 ml of 0.5% bupivacaine after spinalanesthesia administration. The primary outcome was the total 24-hour postoperative fentanyl consumption. Secondary outcomes included pain scores (VAS) at various time points, time to first analgesic request, time to first ambulation, patient satisfaction, and incidence of complications. Results: The PENG group demonstrated significantly lower total 24-hour postoperative fentanyl consumption compared to the QLB group (Mean ± SD: 25.15 ± 5.08 µg vs. 38.45 ± 6.12 µg, p < 0.001).
