Nephron-sparing surgical tactics in patients with renal echinococcosis. Our clinical experiences.
Objectives: The aim of our study is to discuss the methods of diagnosis and treatment of echinococcosis of the genitourinary system, including our experience in the surgical treatment of bilateral renal echinococcosis and perivesical echinococcal cysts. Methods: In this study, we discussed the results of surgical tactics applied in 8 patients (4 women and 4 men) aged 19-53 years diagnosed with renal echinococcosis in the urology department of the Azerbaijan Medical University Hospital between 2003 and 2023. We observed bilateral renal echinococcal cysts in 2 female patients, retrovesical echinococcal cysts in addition to renal echinococcal cysts in 1 male patient, and perivesical echinococcal cysts in addition to bilateral renal echinococcal cysts in 1 female patient. Results: Nephron sparing surgery was chosen in seven patients (87.5%), including partial nephrectomy in 2 patients (3 kidneys), resection of the cyst wall and removal of its contents in 5 patients (6 kidneys), perivesical cystectomy in one patient, resection of the anterior wall of the retrovesical cyst and removal of its contents in one patient. Renal atrophy was detected in one patient as a result of a giant Echinococcosis cyst in the left kidney and nephrectomy (12.5%) was performed. Conclusion: Our clinical experience shows that in most cases of renal echinococcosis, nephron-sparing tactics may be preferred, and the surgical method (partial nephrectomy or cyst wall resection) should be selected based on the results of radiological examination (CT or MRI).
