Simultaneous cardiocerebral infarction complicated by cardiogenic shock
A simultaneous cardio-cerebral infarction (CCI) is defined as both acute ischemic stroke (AIS) and acute myocardial infarction (AMI) concurrently occurring, with an incidence of 0.009%.Early diagnosis and treatment of both conditions are essential, as they require different treatment strategies and are associated with significant mortality and morbidity. In this case, a 67-year-old man presented with AMI of the anterior left ventricular wall concurrently with AIS of 24-hour duration, complicated by cardiogenic shock (CS).The brain imaging revealed the presence of a cerebral posterior circulation occlusion, and coronary angiography identified a lesion in the left anterior descending artery.Following inotropic infusion, the patient recovered from CS and was discharged symptom-free with antiplatelet therapy.Patients with CCI require individualized treatment, and the interventional cardiology department and Stroke Team are vital in determining the mode and sequence of revascularization before the therapeutic time window has passed.
