Clinical effects of PD-1 inhibitor camrelizumab in advanced, recurrent, or metastatic cervical cancer: A meta-analysis

To systematically evaluate the clinical efficacy of the programmed death-1 (PD-1) inhibitor camrelizumab in the treatment of advanced, recurrent, or metastatic cervical cancer (CC). A systematic search was conducted in both Chinese and English databases including PubMed, Cochrane Library, CNKI, Wanfang, VIP up to May 2025 to identify literature on the clinical effects of camrelizumab in advanced, recurrent, or metastatic CC. General information, treatment regimens, and outcome indicators were extracted from the included studies, and their quality was evaluated. RevMan 5.4 and R 4.4 statistical software were used to analyze heterogeneity, and publication bias was evaluated. A total of 13 studies comprising 860 patients were included. The pooled objective remission rate (ORR) after camrelizumab treatment was 0.61 ( 95 % CI = 0.47 ∼ 0.74), which was significantly higher in the experimental group than in the control group ( RR = 1.34, 95 % CI = 1.18 ∼ 1.53, Z = 4.42, P < 0.001). The pooled disease control rate (DCR) was 0.84 ( 95 % CI = 0.77 ∼ 0.89), also significantly higher in the experimental group ( RR = 1.08, 95 % CI = 1.03 ∼ 1.14, Z = 3.09, P = 0.002). After treatment, CA125 levels were significantly lower in the experimental group ( SMD = −2.31, 95 % CI = −2.85 ∼ −1.78, Z = -8.47, P < 0.001), as were SCC-Ag levels ( SMD = −2.21, 95 % CI = −3.26 ∼ −1.16, Z = -4.13, P < 0.001). The overall incidence of adverse reactions after camrelizumab treatment was 0.68 ( 95 % CI =0.53 ∼ 0.80), with no significant difference between the experimental and control groups ( RR = 1.00, 95 % CI = 0.82 ∼ 1.22, Z = 0.03, P = 0.97). The Begg’s test and funnel plot indicated no significant publication bias ( P > 0.05). The PD-1 inhibitor camrelizumab demonstrates significant efficacy in treating advanced, recurrent, or metastatic CC, markedly improving ORR and DCR, reducing tumor marker levels, and exhibiting a favorable safety profile.
