Current Trends in the Development of Stroke Therapeutics and Interpretation of Clinical Trials: A Narrative Review
Stroke remains a leading cause of death and disability worldwide, and despite major advances in reperfusion therapies, unmet needs persist.Intravenous thrombolysis and endovascular thrombectomy have revolutionized acute ischemic stroke care, with expanded time windows and patient eligibility establishing reperfusion as the cornerstone of therapy.Parallel efforts in neuroprotection highlight the complexity of ischemic injury.Agents such as nerinetide, uric acid, butylphthalide, edaravone, nelonemdaz, minocycline and human urinary kallidinogenase illustrate both setbacks and regional successes, underscoring the importance of patient selection, timing, and trial design.Regenerative approaches such as stem cell therapies provide further avenues, though translation to routine care remains challenging.Inflammation is increasingly recognized as a therapeutic target, with insights from cardiovascular trials suggesting potential for agents like colchicine and canakinumab in stroke.Advances in imaging, biomarkers, and digital technologies promise to refine patient stratification and enable precision-guided interventions. The future of stroke therapeutics lies in integration and personalization.Combining reperfusion with neuroprotection, inflammation modulation, and regenerative strategies may overcome the limitations of single-pathway interventions.Adaptive trial designs and global collaboration will be critical to validate these multimodal approaches and ensure accessibility across diverse healthcare systems. In conclusion, while significant progress has been made, the next frontier in stroke therapy will depend on tailoring multimodal, biomarker-driven strategies that extend recovery opportunities to a broader population of patients.
