The purpose of this article is to identify and discuss the development of an outcome measurement system for the evaluation of the life care planning process. A literature review conducted by the authors did not reveal published standardized tools for evaluating life care planning services. In the authors’ view, the importance of outcome data to substantiate the value of a service is important and one sample instrument used by the authors is offered as a model.
