Abstract This case report highlights adenovirus as a rare but significant cause of reactive infectious mucocutaneous eruption, traditionally linked to Mycoplasma pneumoniae . The patient, a young male, presented with profound mucosal inflammation, conjunctival injection, and eventual development of erythema multiforme–like targetoid skin lesions during recovery. Adenovirus was identified as the causative agent via polymerase chain reaction, whereas Mycoplasma was ruled out. The patient responded favorably to corticosteroids, emphasizing an immune-mediated pathogenesis. This case underscores the evolving spectrum of reactive infectious mucocutaneous eruption triggers, the importance of recognizing delayed cutaneous manifestations, and the need for comprehensive diagnostics and multidisciplinary management strategies.
