Defying Age Barriers: Successful Wertheim\'s Hysterectomy for Endometrial Carcinoma in an 84-year-old Hypertensive Woman after Missed Diagnosis: A Case Report

Endometrial carcinoma is among the most common gynecologic malignancies, often diagnosed at an early stage with a favorable prognosis. With increasing life expectancy, the question of whether radical surgery is feasible and safe in very elderly patients is of growing clinical importance. We report the case of an 84-year-old hypertensive woman who presented with 3 months of abdominal pain and postmenopausal bleeding, initially misdiagnosed due to reliance on normal imaging.A biopsy was not possible because of cervical stenosis.Further evaluation with PET-CT confirmed stage III endometrial carcinoma. She subsequently underwent Wertheim's hysterectomy, completed in 90 minutes without complications. The patient recovered rapidly, resumed daily activities within weeks, and remains disease-free at 3 months of follow-up.This case highlights the need for prompt evaluation of postmenopausal bleeding, highlights diagnostic pitfalls in geriatric oncology, and shows that, with careful selection, radical surgery can be safe and beneficial even in octogenarians.
