Simultaneous Diagnosis of Allergic Bronchopulmonary Aspergillosis and Mycobacterium Avium Complex Lung Disease in a Patient with Ankylosing Spondylitis Treated with Adalimumab

We report the case of a 56-year-old male with ankylosing spondylitis who developed allergic bronchopulmonary aspergillosis (ABPA) and Mycobacterium avium complex (MAC) lung disease 3 months after the initiation of adalimumab treatment. The patient presented with a wet cough and nocturnal wheezing. Chest computed tomography revealed bronchiectasis, mucus with a high-attenuation pattern, and granular shadows in the right upper lobe. Simultaneous ABPA and MAC lung diseases were diagnosed on the basis of laboratory tests, bronchoscopic findings, and mucus plug cultures. The patient was successfully treated with oral prednisolone and itraconazole for ABPA and antimycobacterial drugs for MAC lung disease without recurrence for 7 years.
