On stress testing, patient developed classical bidirectional PVCs typical of CPVT (Catecholaminergic polymorphic ventricular tachycardia). The genetic testing showed pathogenic heterozygous variant in CASQ2 gene. Patient was started on propranolol and flecainide and advised to avoid strenuous physical activity.
