Improved Treatment Adherence in Asthma Patients with Beclometasone/Formoterol/Glycopyrronium: 12-Month Results of TriMaximize Study

Background: Treatment adherence (TA) is crucial for achieving the goals of asthma therapy. However, adherence to asthma medication regimens remains suboptimal, leading to poor patient outcomes. Beclomethasone dipropionate/formoterol fumarate/glycopyrronium (BDP/FF/G) fixed triple therapy has demonstrated significant clinical benefits in clinical trials, but real-world evidence on treatment adherence is limited. The TriMaximize study was designed to gain insights into the treatment of BDP/FF/G, including its impact on TA in real-world settings. Methods: Patients were recruited at 154 sites across eight countries (DE, UK, AT, DK, FR, SP, PL, IT). TA was assessed using the Test of Adherence to Inhalers (TAI) with a range from 10 to 50 points (≤45 poor; 46-49 moderate and 50 good adherence). Results: We studied 1445 patients (62.8% female, mean age 57.6 years, 17.9% current smokers). The majority (72.7%) had previously received ICS/LABA (open or fixed) and 27.3% ICS/LABA/LAMA (open or fixed) treatment. Mean TAI score at baseline was 47.0 points, which increased to 47.9 points within 12 months of treatment (p<0.0009). At baseline, 45.7% of patients had good adherence, 28.6% had moderate adherence, and 25.8% had poor adherence. After 12 months, good adherence increased to 54.1%, whereas moderate and poor adherence decreased to 25.8%, and 20.1%, respectively. Overall, 63.5% of patients achieved a higher adherence category or maintained good adherence and were classified as responders. Conclusion: Significant improvement in treatment adherence was observed in patients with moderate to severe asthma over a 12-month period of fixed triple BDP/FF/G therapy.
