A rare case of exogenous fungal endophthalmitis following a traumatic corneal insult

To report a case of exogenous endophthalmitis caused by Rhizomucor miehei after traumatic corneal injury. A 70-year-old gentleman with a recent history of trauma to the left eye involving a barbed wire two months previous presented with reduced visual acuity amid significant ocular pain. He was initially treated with broad spectrum antibiotic therapy including ceftazidime 5% and vancomycin 1% and the antifungal agent voriconazole. The subsequent vitreous culture was positive for Rhizomucor miehei. Subsequent diagnostic vitrectomy was undertaken and the patient was followed- up in clinic therafter. An accurate diagnosis for fungal endophthalmitis is elusive to many clinicians, owing to its subtle and sometimes misleading clinical presentation. An open mind accompanied with a high index of clinical suspicion for fungal pathology is essential in managing such presentations.

Abstract

To report a case of exogenous endophthalmitis caused by Rhizomucor miehei after traumatic corneal injury. A 70-year-old gentleman with a recent history of trauma to the left eye involving a barbed wire two months previous presented with reduced visual acuity amid significant ocular pain. He was initially treated with broad spectrum antibiotic therapy including ceftazidime 5% and vancomycin 1% and the antifungal agent voriconazole. An accurate diagnosis for fungal endophthalmitis is elusive to many clinicians, owing to its subtle and sometimes misleading clinical presentation. An open mind accompanied with a high index of clinical suspicion for fungal pathology is essential in managing such presentations.
