CONCERNING HIGH RATE OF ASEPTIC LOOSENING IN SPECIFIC COMPONENT COMBINATIONS OF A POPULAR TOTAL KNEE ARTHROPLASTY IMPLANT BRAND: AN INTERNATIONAL MULTICENTRE RETROSPECTIVE COHORT STUDY
Emerging evidence from the UK National Joint Registry (NJR) has demonstrated a concerningly high rate of revision for aseptic loosening in specific tibia and femur component combinations of a popular brand of total knee arthroplasty (TKA). This international multi-center retrospective cohort study compares clinical outcomes for the various component combinations for this popular TKA brand. This was an international multi-center retrospective cohort study assessing all combinations of the popular TKA implant brand. There were 9,392 primary TKA components included with 1,744 ‘high-risk’ component combinations and 7,648 ‘standard-risk’ combinations reported. The median time to follow-up was 7.4 (IQR 4.8 – 9.4) years in the high-risk and 10.4 (IQR 7.5 – 13.8) years in the non-risk group. The primary outcome variable was revision for aseptic loosening. All-cause revision was a secondary outcome variable. Multivariate regression analysis was performed. Cox regression and Kaplan-Meier (KM) survival analysis was used to determine survivorship free of revision for aseptic loosening. The high-risk component combinations had significantly higher rates of revision for aseptic loosening (4.9% v 0.4%, p This popular TKA brand has a concerning high revision rate for aseptic loosening with certain component combinations. These findings may lead to the eventual withdrawal of these specific implant component combinations from the market.
