AMNIOTIC FLUID EMBOLISM WITH ONSET OF ALLERGIC RASH AND VISION LOSS: A CASE REPORT

Abstract Objectives Amniotic fluid embolism (AFE) is a rare but catastrophic severe obstetrics complication during delivery and labour. AFE with onset of allergic rash and vision loss occurring in the initial period of the event has never been reported previously. Subjects and Methods We report here the case of a 34-year-old Chinese woman diagnosed with obstetric antiphospholipid syndrome (OAPS), presenting sudden allergic rash and vision loss followed by coagulation dysfunction and disseminated intravascular coagulopathy (DIC) after rupture of membranes during labor. Interestingly, she had normal blood pressure and oxygen saturation as allergic rash and vision loss occurred in the initial period of this devastating event. Results The coagulopathy fully resolved at three hours postoperation. On postoperative day one, her condition markedly improved, and no obvious bleeding from the incision and vagina, she had normal vision and fundus examination was normal. The patient was discharged to home the eighth day after the operation. The patient was successfully managed by timely hysterectomy and massive blood product administration. Conclusions AFE is often unpredictable, unpreventable and devastating, this case demonstrates that the syndrome can also be varied and atypical. Timely recognition, aggressive operation, adequate blood product transfusion and multidisciplinary teamwork are particularly important for a successful outcome.
