Vancomycin-specific Urinary Immunostaining for Noninvasive Screening of Vancomycin-associated Cast Nephropathy

Abstract
Vancomycin is a widely used antibiotic and a known cause of acute kidney injury (V-AKI). Recent studies suggest that vancomycin can form obstructive aggregates with uromodulin in kidney tubules, contributing to acute kidney injury (V-AKI). However, detecting these aggregates typically requires kidney tissue for immunostaining. Urine-based vancomycin immunostaining has been proposed as a non-invasive alternative for diagnosing V-AKI, but its specificity remains uncertain.
