One Pillar, Three Cusps
A 54-year-old man (81.5 kg, 186 cm) with hypertension and peripheral arterial disease presented with progressively worsening exertional chest pain. Coronary angiography revealed occlusion of the left anterior descending and right coronary artery; transthoracic echocardiography (TTE) demonstrated infero-basal hypokinesia, a left ventricular ejection fraction of 45%, and a mobile echo-dense mass within the right ventricle (RV), which was originally interpreted as a thrombus (Fig. 1, Video 1). A subsequent study showed that the mass appeared to have resolved spontaneously.
