Evaluation of Long-term Efficacy of 5-ALA-PDT for Oral Potentially Malignant Disorders: A Cohort Study

To evaluate the long-term efficacy of 5-aminolevulinic acid photodynamic therapy (5-ALA-PDT) in oral potentially malignant disorders (OPMDs). A prospective cohort study was conducted on 84 OPMD patients treated with 5-ALA-PDT, followed up for at least 1 year for recurrence and malignant transformation. A matched retrospective control group of untreated patients was also included. The median follow-up was 38 months, with a total recurrence rate of 51.1% and a malignant transformation rate of 30.9%. Multivariate analysis showed that non-smokers and those without post-treatment swelling had higher recurrence rates. Female gender, baseline pain symptoms, and lesion recurrence were linked to a higher risk of malignant transformation. Kaplan-Meier analysis indicated faster cancer onset in the control group (P=0.041). 5-ALA-PDT effectively delays malignant transformation in OPMDs, with better outcomes in high-risk lesions and certain patient characteristics.
