Evaluation of the effectiveness of photodynamic therapy in patients with high-grade vulvar intraepithelial neoplasia (VIN III) using the drug based on 5-aminolevulinie acid

High-grade vulvar intraepithelial neoplasia (VIN III) poses a significant risk of progression to vulvar cancer, making effective treatment crucial. Photodynamic therapy (PDT) using a 20% 5-aminolevulinic acid (5-ALA) solution offers a non-invasive, organ-preserving alternative to conventional surgical methods. A study of 46 patients with morphologically confirmed VIN III was conducted. PDT involved topical application of 20% 5-ALA solution with 3-hour exposure, followed by laser activation at 630 nm using Biospek equipment. Each patient underwent two courses of PDT, with three sessions per course, at one-month intervals. Three months post-treatment, clinical examination and vulvoscopy showed no atypical sites in treated tissues. Cytological evaluation revealed regression to NSIL in 42 patients (91.3%). PDT with 5-ALA is an effective, minimally invasive treatment for VIN III, achieving high therapeutic success and providing an organ-preserving option for patients.

PDT with 5-ALA is an effective, minimally invasive treatment for VIN III, achieving high therapeutic success and providing an organ-preserving option for patients.
