Itraconazole combined with photodynamic therapy for sporotrichosis: a case report

A 73-year-old female patient developed a rash on her right hand more than 3 months ago, which gradually spread to her right forearm with pain. Biopsy pathology in another hospital showed suppurative inflammation of the dermis and subcutaneous tissue, local fat necrosis and granulomatous inflammation, negative PAS, negative acid-fast staining. There was no obvious improvement after symptomatic treatment, the symptoms gradually worsened. After arriving at our department, the bacteria culture, the identification of non-tuberculous mycobacterium and the fungal culture were negative. A new rash was taken for histopathological consideration of infectious granuloma. The patient had a retrospective history of injury while doing farm work. Diagnosis: Sporotrichosis. Treatment: Oral itraconazole 0.1g bid combined with 5-aminolevulinic acid photodynamic therapy (ALA-PDT) for 3 times, the rash was significantly resolved.
