Guided endodontic access and treatment of a calcified mandibular Canine: A case report
Pulp canal calcification (PCC) poses a significant challenge in endodontics, complicating canal localisation and increasing the risk of procedural errors. Guided endodontics, a CBCT-based digital workflow, has emerged as a precise and minimally invasive solution for such cases. A 36-year-old female patient presented with a symptomatic, non-responsive mandibular right canine, with radiographs confirming PCC. CBCT data (Orthophos SL, 5×5 cm volume, 80 μm voxel size) and a digitised impression were merged using SICAT Endo software for virtual treatment planning. A 3D-printed guide (fabricated by SICAT) facilitated precise access cavity preparation with a 1.2 mm spiral carbide bur. The guided approach enabled accurate canal localisation with minimal dentin removal. An analysis of available solutions (e.g., CoDiagnostix, BlueSky, SimPlant) identified SICAT Endo as the only software dedicated specifically to endodontic applications. This case demonstrates that guided endodontics is a safe, predictable, and minimally invasive technique for managing teeth with severe PCC, offering a significant advantage over conventional methods. This digital workflow offers significant value to General Dental Practitioners (GDPs), particularly when managing complex endodontic cases.
