Early Maxillary Expansion-Induced Bone Remodeling Guides Permanent Teeth Eruption.
This study focuses on early arch expansion treatment for maxillary arch stenosis during the mixed dentition period. It investigates the synergistic effects of this treatment on permanent dental development and jaw growth. The research also evaluates the comprehensive intervention outcomes of early expansion on dental and maxillofacial health management. A 7-year-old female patient presented with Angle Class III malocclusion and skeletal Class II tendency (SNA 82.1°, SNB 76.9°, ANB 5.2°).She presented with maxillary stenosis, dental crowding, and crossbite involving teeth 11 and 41.Panoramic imaging showed tooth germ 24 was horizontally impacted at a 90° angle relative to tooth 23, with a 73.6% impaction risk. Treatment included two phases: Phase 1 used a removable Hyrax expander to achieve 3 mm maxillary expansion. Phase 2 involved extracting retained deciduous tooth 64 and maintaining expansion until tooth 24 erupted. Oral health education was provided throughout the 6-month intervention period. After 6 months,3D analysis showed improved arch form and aligned incisors. Cephalometric measurements revealed a 1.2° ANB reduction. Panoramic imaging demonstrated tooth 24 positioned distally to tooth 23 with mesial inclination reduced from 90° to 15°, indicating normalized eruption. Early arch expansion adjusted eruption paths of 24 potentially obstructive tooth germs from horizontal to near-vertical. It reduces surgical risk from 73.6% to 12.3%. A 7-year-old child's 6-month intervention prevented orthodontic traction and implant surgery, validating the three-tiered prevention system's clinical feasibility during occlusion development.
